PESSI Card (R-5)
Description
Official registration card issued by PESSI to secured workers in Punjab. The R-5 card serves as proof of social security registration and grants workers and their dependents access to free medical treatment at PESSI hospitals, cash benefits during sickness and maternity, disability pensions, survivor pensions, and other statutory benefits.
✅ Eligibility Criteria
Worker must be employed in an industrial or commercial establishment registered with PESSI in Punjab; wage must be at or below the ceiling notified by PESSI (currently up to PKR 40,000/month for registration eligibility, subject to revision); employer must be registered with PESSI and file monthly C-1 contributions; contract, casual, and seasonal workers may also be covered if they meet wage and establishment criteria
📋 Documents Required
Worker’s CNIC copy; Dependents’ CNIC/B-Form (spouse, parents, children above 18 years); Two passport-size photographs of worker; Medical Fitness Certificate; Detailed Information Form submitted to nearby PESSI office
